女性METH成瘾者脑效应连接特征及12周有氧训练干预实证研究

摘要: 目的:研究女性甲基苯丙胺(Methamphetamine,METH)成瘾者大脑效应连接(Effective Connectivity, EC)特征及12周有氧训练对成瘾者毒瘾渴求、情绪等及大脑效应连接的干预效果。方法:受试者随机分为对照组(CON组)、METH成瘾组(METH组)及METH成瘾运动组(METH-Ex组),采用功能性近红外光谱技术(functional near - infrared spectroscopy , fNIRS)进行脑血氧信号采集。受试者睡眠、抑郁、焦虑及渴求评估分别采用PSQI、SDS、SAS、DSQ量表进行,METH组与CON组测试分为静息态测试和运动态测试,METH-Ex组进行12周中等强度有氧搏击操运动干预,并分别在干预1周、6周、12周进行静息态脑血氧检测及情绪、渴求等评估。脑血氧信号通过贝叶斯推论进行数据转换,效应连接根据Shapiro-Wilk进行检验,利用SPSS 20.0进行数据重复测量方差分析。结果:METH成瘾和有氧搏击操干预对LPFC-LMC、RPFC-LMC之间大脑效应连接存在显著交互。静息态METH组大脑EC耦合强度在LPFC-RPFC、LPFC-RMC显著低于CON组;运动态METH组大脑EC耦合强度在LPFC-RPFC、RPFC-LPFC、LPFC-LMC、LPFC-RMC、RPFC-LMC、RPFC-RMC、LMC-RMC显著低于CON组,且运动态RPE与LPFC-RPRC、RPFC-LPFC的EC水平呈负显著相关。12周有氧搏击操干预SDS、SAS、DSQ组内结果均存在显著性差异(P<0.01),受试者抑郁、焦虑、渴求水平显著下降。METH-Ex组脑区EC耦合强度在LPFC-RPFC、RPFC-LPFC呈现降低趋势,实验后大脑EC耦合强度在LMC-RMC、LMC-LPFC、LPFC-RMC、RPFC-RMC、RMC-RPFC呈上升趋势,且SDS与LMC-LPFC、LMC-RPFC效应连接水平具有显著性的中等强度相关,SAS与LPFC-RPFC效应连接水平具有显著性的中等强度相关,DSQ与LPFC-RPFC、RPFC-LPFC等脑区效应连接水平呈弱相关性。结论:女性METH成瘾者大脑效应连接低于正常女性人群,单次有氧运动干预有降低运动态EC水平的趋势,有氧干预后RPE量表评分与EC变化呈负相关关系;持续有氧搏击操干预女性METH成瘾者EC水平表现出先下降再上升趋势,显示长期运动干预对女性METH成瘾者可能具有更好的效果;有氧搏击操运动干预显著改善女性METH成瘾者抑郁、焦虑及对METH的渴求,且这些改善作用于部分脑区间效应连接相关。

Abstract: Objective: To study the characteristics of effective connectivity (EC) in female methamphetamine (METH) addicts and the intervention effect of 12-week aerobic training on addiction thirst, mood, and brain EC in addicts. Methods: The subjects were randomly divided into control group (CON group), METH addict group (METH group) and METH addict exercise group (METH-Ex group). Functional near-infrared spectroscopy (fNIRS) was used to collect cerebral blood oxygen signals. Sleep, depression, anxiety and thirst in subjects were assessed respectively by PSQI, SDS, SAS and DSQ scales. Both the METH group and the CON group received resting and exercise tests, and the METH-Ex group received a 12-week aerobic kickboxing training intervention of moderate intensity, and resting cerebral oxygenation tests, and mood and craving assessments were performed at 1st, 6th and 12th week. The cerebral blood oxygen signal data were transformed by Bayesian inference, the EC was tested according to Shapiro-Wilk, and data were analyzed by repeated measures ANOVA using SPSS 20.0. Results: There was a significant interaction between METH addiction and aerobic kickboxing exercise intervention on brain EC between LPFC-LMC, RPFC-LMC. The intensity of brain EC coupling in the resting METH group was significantly lower in LPFC-RPFC and LPFC-RMC than in the CON group; the intensity of brain EC coupling in the METH-Ex group was significantly lower in LPFC-RPFC, RPFC-LPFC, LPFC-LMC, LPFC-RMC, RPFC-LMC, RPFC-RMC, LMC-RMC than in the CON group, and RPE-Ex was negatively and significantly correlated with EC levels in LPFC-RPRC, RPFC-LPFC. Results of 12-week aerobic kickboxing exercise intervention in SDS, SAS, and DSQ groups were significantly different (P<0.01), and the level of depression, anxiety, and craving in subjects decreased significantly. The intensity of brain EC coupling in the METH-Ex group decreased significantly in LPFC-RPFC and RPFC-LPFC, while the intensity of brain EC coupling in LMC-RMC, LMC-LPFC, LPFC-RMC, RPFC-RMC, RMC-RPFC showed an increasing trend after the experiment. SDS had a significant moderate intensity correlation with the EC level of LMC-LPFC and LMC-RPFC, and SAS had a significant moderate intensity correlation with LPFC- RPFC, and DSQ showed a weak correlation with LPFC-RPFC and RPFC-LPFC. Conclusion: Brain EC in female METH addicts is lower than that in normal females, a single aerobic exercise intervention tends to reduce the EC level of METH addicts in exercise state, and RPE scale score is negatively correlated with EC change after the aerobic intervention; EC level in female METH addicts shows a decrease-increase trend after continuous aerobic kickboxing exercise intervention, indicating that long-term exercise intervention may have better effect on female METH addicts; aerobic kickboxing exercise intervention significantly lowers the level of depression, anxiety and METH craving in female METH addicts, and these changes are correlated with the EC between some cerebral regions.
