高跟鞋对女大学生行走时膝关节力矩的影响

The Effect of High Heel on Knee Joint Torques during Gait

摘要: 目的: 通过逆向动力学法计算健康女大学生赤足与穿高跟鞋2种状态下行走时膝关节力矩,分析穿高跟鞋是否显著增加膝关节负荷,探讨穿高跟鞋后女大学生走路机制的改变规律。方法: 将Vicon系统捕捉的运动学参数,AMTI测力台采集的动力学参数,结合人体环节参数,输入逆向动力学公式计算三维膝关节力矩,比较2种状态的异同。结果: (1)支撑阶段的前80%时相,穿高跟鞋增加19%屈曲力矩,增加39%伸展力矩。多出的屈曲与伸展力矩增加了股四头肌拉力负荷,并通过髌骨上方韧带以压力形式施加给髌股关节;最后蹬离地面时相,穿高跟鞋减少21%屈曲力矩。大幅减少屈曲力矩可能会减弱支撑腿最后蹬离地面的推力。(2)穿高跟鞋提前了所有内收力矩峰值出现时相,并增加9%第1力矩峰值和5%第2力矩峰值。增大内收力矩会增加膝关节内侧关节囊负荷。这种补偿性牵拉可能会破坏膝关节原有的平衡控制能力。(3)穿高跟鞋使外旋力矩到内旋力矩过度时相提前12%,并增加18%外旋力矩和11%内旋力矩。蹬离地面瞬间是穿高跟鞋人群维持身体竖直体态的最薄弱阶段,即关节损伤可能多发生在此时期。(4)穿高跟鞋增大了前进方向的制动力,减小了推动力。结论: 高跟鞋显著性的改变了健康女大学生行走时膝关节原有力矩特征。

Abstract: Objective: To analyze whether high-heeled shoes can significantly increase the knee joint load by computing through Inverse Dynamic the knee joint torques of college girl students with bare foot and high-heeled shoes during walking and investigate the change in walking pattern of college girl students with high-heeled shoes. Methods: Based on data from Vicon, AMTI and personal segment parameters, compute and compare through the Inverse Dynamic equations the knee joint torques in 3D and compare the statistics in the 2 conditions. Results: (1) High-heeled shoes increase the flexion torque by 19% and extension torque by 39% during first 80% of the supporting phase. Redundant torque increases the tension from quadriceps femoris and results in more pressure on patella-femoral joint. High-heeled shoes decrease the flexion torque by 21% at the leaving phase. Greatly reduced flexion torque may weaken the push power directly during the last phase of single stance. (2) High-heeled shoes change the abduction torque ahead of time and peak value by 9% and 5% respectively. Increased abduction torque may add the load on interior compartment of knee joint. This compensatory tension could destroy the balance control of knee joint. (3) High-heeled shoes change the transit from external to internal torque by 12% ahead of time, increase external torque by 18% and internal torque by 11%. The lasting leaving phase may be the weakest period for body alignment of high heel wearer, namely, there is a big probability for joint injury. (4) High-heeled shoes increase breaking force and decrease propulsion force. Conclusion: High heel shoes significantly change the normal torque pattern of knee joint during healthy female's gait.
